What’s Actually Happening in Your Brain

Depression, anxiety, and PTSD aren’t just feelings. They change the brain in real, physical ways.

When stress goes on for a long time, the tiny connection points between brain cells (called dendritic spines) start to shrink. This happens in the brain areas that handle focus, mood, and hope (the prefrontal cortex and hippocampus). That’s part of why those things get harder to reach over time. It isn’t a flaw in who you are. It’s a change in how the brain is wired.

Ketamine helps rebuild those connections (synapses). It blocks a receptor called NMDA, which sets off a chain that raises a growth factor called BDNF and helps new connections form. In lab studies, this began within hours of a single dose — a real, physical change, not just a shift in mood (Li et al., Science, 2010).

That’s what we mean by “the window.” It’s real, not just a figure of speech. For a short time after each infusion, the brain can build and strengthen connections better than it could the week before.

Here’s the part most people are never told: the open window (a period of high plasticity) doesn’t decide what gets built. Your brain strengthens whatever you practice during that time — even old patterns, if nothing changes. New, healthier patterns don’t build themselves. Infusion Day — Rest

Your brain is doing work you cannot feel yet. No driving, no major decisions, no hard conversations. Low input. Quiet. Sleep is productive here.

Days 1–3 — Peak Plasticity

The highest-leverage days of the entire series. Small actions land differently now. The walk. The text you have been avoiding. The boundary you have needed to set. This is when to do the next right thing — even if your mood has not caught up yet.

Days 4–7 — Consolidation

Between Infusions

Questions About Integration

How long does the neuroplasticity window actually last?

Research on ketamine’s effect on synaptic plasticity points to changes beginning within hours and the most pronounced effects in roughly the first several days after an infusion. Clinically, we treat the first 24 to 72 hours as the highest-leverage window, with continued benefit through the first week. Because you receive a series rather than a single dose, these windows stack — which is a large part of why the series of six matters.

Do I have to do the integration work for ketamine to help me?

Many patients feel meaningful relief from the infusions alone. The honest answer is that relief and durability are two different questions. Ketamine reliably opens the window; what fills that window is what tends to determine how long the change holds. Integration is how you turn a good few weeks into a different baseline.

What if I don’t feel like doing anything after my infusion?

That is expected, and it is the entire point of how we teach this. Your mood will keep making its case for staying on the couch — that pattern took years to build. Integration is not about waiting to feel motivated. It is about following through on the thought anyway, at the smallest possible scale. Five minutes. One walk. The next right thing, not everything.

Is Brain Health Reboot an extra cost?

No. Brain Health Reboot is included with every IV ketamine series at AlphaOmega Wellness. It is not an upsell — it is part of how we practice. ROOT is our ongoing program for patients who want to keep building after the series ends.

Is integration the same thing as therapy?

No. Brain Health Reboot and ROOT are educational, not therapy, and they do not replace a therapist. They teach the neuroscience of what is happening in your brain and give you practical skills drawn from CBT, DBT, and ACT frameworks that you can use between sessions. If you have a therapist, this works alongside them.

Who supervises the medical side of treatment?

Every infusion is conducted under the medical direction of Dr. Dee Bonney, MD — a board-certified emergency physician with 25+ years of experience — with a paramedic present at all times. Our staff aren’t just ACLS-trained. They’re working paramedics. That card isn’t gathering dust in a wallet — these are people who use those skills regularly in the field. Vital signs are monitored continuously throughout every session.
